Is Sentinel Lymph Node Mapping Indicated for Isolated Local and In-Transit Recurrent Melanoma?

Abstract: LM/SL can accurately identify SNs draining an RM, and the high rate of SN metastases and associated poor disease-free survival for patients with tumor-positive SN suggests that LM/SL should be routinely considered in the management of patients with isolated RM.

“…Although in-transit disease has a high probability of clinically occult regional nodal involvement and a positive sentinel node in the presence of in-transit metastasis portends a more ominous prognosis, the impact of SLNB on outcome remains unknown. 97 For patients for whom resection is not feasible, previous resections have been unsuccessful, or who refuse surgery, nonsurgical local approaches for treating stage III in-transit melanoma include intralesional injections, local ablation therapy, topical imiquimod, and RT. Intralesional Injections: A variety of agents have been tested as intralesional injections for melanoma.…”

“…Repeat SNB has been used successfully in patients with a local relapse of melanoma and breast cancer [12][13][14]. This study shows that repeat SNB is feasible in locally recurrent penile carcinoma.…”

“…Clinicians seeking to determine prognoses for lymph node metastases first concentrated on developing methods of improving their gross identification of cancer in resected specimens, thereby improving likelihood of finding more metastatic deposits in lymph nodes. The sentinel node concept appears to fit to this search, having been validated as a prognostic tool and one of the most important methods for the multidisciplinary management of breast cancer and melanoma [1][2][3][4][5][6]. In contrast to breast carcinoma and malignant melanoma, where the technique is used as a determinant of the need for regional lymphadenectomy, in colorectal cancer lymphadenectomy remains a vital component of the oncological operation.…”
