Is the Cluster Headache Local Mastocytic Diaethesis?

Abstract: SYNOPSIS Mast cell counts from the skin over painful regions were obtained in cluster patients and compared to those from controls. A significantly higher count was found in the cluster group, and suggests a possible causal relationship.

“…On the basis of our own material (8), we can support these observations. Cuypers et al (17) failed to confirm Appenzeller's (15,16) as well as our own (6,7,8) data. One possible explanation for these discrepancies may be the times at which the biopsies were done.…”

“…Another aspect of CBH is the presence of fibrin deposits accounting for the induration characteristic of these reactions (18). Our previous results however (7) failed to show any effector cells participating in immunologically-mediated skin reactions. In this respect, the proliferation of mast cells with the features of dissolution seemed to be an isolated phenomenon.…”

“…It is encouraging to find that studies are now being conducted by several groups on the changes of the background terrain and of the local areas of headache. 16,17,18,19,20 This brings me to a consideration of migraine's connections with stress. There is much clinical observation to support the idea that the migraineur handles stressful events in an exaggerated or inappropriate manner.…”

“…According to some reports, an increased number of mast cells was observed in the skin of the temporal region in cluster headache. 10 Electron microscopy showed that these mast cells become active during attacks of headache. 11 No mast cell membrane stabilizing drugs have so far been administered in the treatment of cluster headache.…”
