Is the Presence of a Solitary Kidney an Independent Risk Factor for Acute Kidney Injury after Contrast-enhanced CT?

Abstract: Our study did not demonstrate any significant differences in the rate of AKI, dialysis, or death attributable to contrast-enhanced CT in patients with a solitary kidney versus bilateral kidneys.

“…Of interest are the data on the demographics of their SFK cohort, which shows hypertension in 65% and CKD in 39%. This was significantly higher than the control cohort with 47% and 10%, respectively [ 80 ], highlighting the potential health issues in any individual with an SFK.…”

“…McDonald et al studied the risk of contrast-induced acute kidney injury in a cohort of 264 SFK patients, of whom at least 80% had a nephrectomy in adulthood [ 80 ]. Their analysis of the risk with a propensity score-matched control group did not show an increased risk of acute kidney injury in the SFK cohort.…”

“…Are Patients with a Single Kidney at Increased Risk for CA-AKI or CI-AKI Beyond That Associated with Their eGFR? Patients with a single normal or partially functioning kidney (eg, kidney agenesis, nephrectomy, transplant) should be managed similarly to patients with normal kidney volume (eg, two normal kidneys) (15,48). In patients with a single normal or partially functioning kidney, clinical risk should be determined based on overall kidney function (ie, eGFR) and clinical circumstances (ie, AKI).…”

The presence of a solitary functioning kidney should not influence decision making regarding the risk of CA-AKI or CI-AKI (15,48).…”

“…McDonald et al retrospectively examined 6,175 patients who received contrast-enhanced CT in a single facility. 68 Propensity score (1:3) matching was performed to adjust for basal renal function and underlying diseases, and 247 patients with solitary kidney and 691 patients with bilateral kidneys were compared. Enhanced CT was performed 30 days after nephrectomy (76% due to malignant tumor; 5% in transplant recipients; 19% in others).…”
