Is there “brain OAB” and how can we recognize it? International Consultation on Incontinence‐Research Society (ICI‐RS) 2017

Abstract: Future research should aim to identify the pathogenesis of WMD and the aging processes affecting the brain and the bladder, with possible benefits in prevention strategies, as well as connectivity disorders within the CNS, the pathophysiology of OAB in childhood and the neurochemical pathways connecting affective disorders with OAB.

“…The difference in supraspinal activity between OAB patients with or without DO was confirmed by Tadic et al [37], demonstrating that older age and a greater burden of white matter damage in patients with DO are associated with more severe functional urinary impairment. Several reports support the hypothesis that this "white matter disease" could be the anatomical substrate for the brain aetiology of OAB associated with DO, maybe through frontal hypoperfusion [38].…”

“…iv) The supraspinal theory: urgency has its origin in the brain and brainstem. With age, deterioration in certain segments of the white matter is possible, leading to urinary disorders (38). v) Detrusor underactivity: secondary to urothelial or suburothelial dysfunction or from detrusor muscle dysfunction, accompanied by detrusor underactivity appears urgency, which has been found in many studies [due in particular to recurrent urinary tract infections (UTIs) that occur through chronic urinary retention] (22,39).…”

“…Although OAB is a widespread problem, the pathophysiology is still unclear. It is likely multifactorial but increasing recent evidence supports a central role of disturbed brain and bladder connection [ 3 , 4 ]. Previously, everything was concentrated only around the bladder.…”
