Is there treatment for traumatic optic neuropathy?

Abstract: TON may lead to profound visual disability. Because of the absence of Class I evidence relating to treatment of TON, a lack of consensus exists among clinicians regarding the appropriate treatment for TON. Our review of the recent literature finds that there is still no reliable evidence that treatment of TON with corticosteroids, surgical optic canal decompression or both confers any benefit over observation alone, and yet these interventions incur an additional risk that may not warrant their routine use.

“…[16][17][18] TON causes permanent visual loss and there are currently no effective treatments to preserve or restore vision. 19,20 After blunt ocular trauma, initiator caspase-9 is activated and initiates localized photoreceptor death, which can be attenuated in the lesion penumbra of animal models by caspase-9 inhibition. 21 However, the mechanisms of the accompanying RGC death in this model have not been defined and no treatment has been shown to neuroprotect RGCs after blunt ocular trauma.…”

“…The treatment for glaucoma is based on drugs and surgery that reduce intraocular pressure, whereas there is no treatment for NAION, nor to reverse the process nor for its recurrence[32]. Traumatic optic neuropathy is a cause of severe visual loss and it has no reliable treatment[33]. Neuromyelitis optica, also known as Devic’s disease, is an autoimmune, demylienating disorder which causes optic neuritis.…”
