Is tumour size a contraindication to laparoscopic adrenalectomy? Case report.

Abstract: The authors present a case report of a patient with a large, hormonally silent tumour of the right adrenal gland. Due to the patient's numerous strains, the necessity of two gynaecological operations, and treatment of broken bones, adrenalectomy was contraindicated for 2 years. After that time, the size of the tumour reached 18 cm × 12 cm. The patient was selected for laparoscopic adrenalectomy, which was successful. The size of the tumour and performed abdominal surgery did not constitute substantial obstacle… Show more

“…In our department, LTA was the only technique of minimally invasive adrenalectomy used in the analyzed period. Originally, laparoscopic adrenalectomy was indicated in patients with adrenal tumors < 5 cm in diameter, but it is now commonly used in patients with tumors > 8 cm [ 11 , 12 ]. In our study, the largest adrenal tumor removed during LTA was 9 cm in diameter.…”

“…Some authors consider a diameter of ≥6 cm a relative contraindication to laparoscopic adrenalectomy [3,4]. On the other hand, there are recent publications where authors report the feasibility of laparoscopic adrenalectomy of large tumors [15,16,17,18,19,20]. …”

“…Moreover, there are reports of high rate of intraoperative complications [14]. However, tumor size as a limitation for laparoscopic adrenalectomy is questioned by some authors [15,16,17,18,19,20]. …”

“…Clinical experience shows that the presence of a malignancy with diameter greater than 10 cm qualifies for laparoscopy, but there must be no infiltration of the tumour into neighbouring tissues [7]. In the case of larger, benign tumours, not hormonally active, laparoscopic adrenalectomy in technically possible.…”

“…In literature there are many publications comparing both methods and their complications, radical oncological therapy, duration of the operation, blood loss, and hospitalisation time. In most cases, the laparoscopic method seems to be better than the open procedure [7,8]. The surgical technique must be adjusted to the patients' needs.…”
