Ischemia of the Internal Capsule due to Mild Head Injury in a Child

Abstract: We encountered an instructive case of repetitive reversible severe neurological deficit due to ischemia of the internal capsule after mild head injury. A 1-year-old boy fell and hit his head on the floor without losing consciousness. Intermittent episodes of left hemiparesis lasting from 30 s to 30 min developed 4 h later. Magnetic resonance (MR) imaging revealed acute infarction in the left internal capsule and corona radiata on diffusion-weighted imaging, and no microbleeding on susceptibility-weighted imagi… Show more

“…From clinical data presented in the literature, ischemic stroke preceded by mild head injury has been described in pediatric patients [35,36,37,38]. Most of the patients reported in the literature had ischemic stroke after a head injury within several hours or days without the presence of artery dissection.…”

“…The pathogenesis of AIS after mild trauma is less clear and may be related to the vulnerable cerebral anatomy present in childhood in combination with an additional risk factor, such as prior CNS infection, resulting in a milieu where subtle shear or stretch can result in intimal lesions and thrombosis [53, 54]. A subset of children may be susceptible to vertebral artery dissection and posterior circulation stroke after minor trauma due to congenital bony and ligamental abnormalities.…”

“…These associations are also observed in children who are victims of inflicted TBI (iTBI; Adelson et al, 2005;Vavilala et al, 2007). Vasospasm after TBI has also emerged as having significant importance in some cases, although reports in pediatric TBI are limited (Nabika et al, 2007;Ojha et al, 2005). However, biochemical or molecular mediators of post-traumatic CBF reductions in infants and children remain to be defined.…”
