Ischemic Colitis

Abstract: Most clinicians associate ischemic colitis with elderly patients who have underlying cardiovascular comorbidities. While the majority of cases probably occur in this population, the disease can present in younger patients as a result of different risk factors, making the diagnosis challenging. While a majority of patients respond to medical management, surgery is required in approximately 20% of the cases and is associated with high morbidity and mortality.

“…The marginal artery of Drummond protects the colon from ischemia by forming a system of arcades connecting the major arteries and the meandering mesenteric artery (arc of Riolan) ( 1 ). Anatomically, two areas of the colon are considered more vulnerable for ischemia: The first vulnerable area is the splenic flexure (Griffith’s point).…”

“…The classic symptomatology of IC consists of abdominal pain, hematochezia, and leucocytosis in an elderly patient ( 1 ). These symptoms vary considerably and depend mainly on the degree of the ischemia and the length and the thickness of the colon that is affected.…”

“…Ischemic colitis (IC), also known as colon ischemia, is the most common type of intestinal ischemia caused by damage to the colonic wall resulting from reduced blood flow [1]. This can vary from superficial injury of the mucosa and submucosal layer to full-thickness necrosis of the colonic wall [2]. Most instances are transient and resolve spontaneously with hospitalization and expectant management.…”
