Isolated Penile Fournier's gangrene: A very rare entity

Abstract: Comparatively to scrotal gangrene, isolated penile gangrene is very rare due to the rich blood supply of the organ. It is thought to be initiated by a traumatic or vascular insult to the penis. This condition requires parenteral antibiotic therapy and serial debridement of necrotic tissue. Split thickness skin graft is thought to be the best approach to cover penile skin loss. We share our experience on the presentation of an isolated penile gangrene in a 35-year-old male. In the light of this case, we review … Show more

“…Objects that cause penile constriction can be classified into hard materials, such as metal rings and plastic bottles, and soft materials such as rubber bands, threads, and strings 1 , 3 , 4 . The removal of soft constraining objects often requires more time, and a higher incidence of serious complications is observed, including necrosis 5 , 6 . This is attributed to their smaller surface area of constriction, intense pressure due to contraction, and often delayed identification and extended duration of constriction, as they can become embedded in the tissue.…”

“…It has also been reported to follow calciphylaxis of the penis. This is a rare condition in which there is calcium deposition in small and medium sized vessels of the skin causing ischemic necrosis associated with focal thrombosis, and intimal fibroblastic proliferation with luminal narrowing and occurs mainly in patients with end-stage renal disease and diabetes [2,4].…”

“…67 Early diagnosis of FG patients without risk factors and inducing conditions is very difficult, which require clinician to sufficiently understand the early manifestations. 37,68 The earliest clinical manifestation of skin in FG patients includes perineum and perianal pain, pruritus, edema and unclear boundary patchy erythema. As the infection continues to spread along the fascia plane at the speed of 1 inch/h, • Alcohol abuse 42 • HIV 43 • Leukemia 44 • Chemotherapy 45,46 • Liver disease 16 • Atherosclerosis 38…”
