Isolated presumed optic nerve gumma, a rare presentation of neurosyphilis

Abstract: PurposeThe incidence of syphilitic infections continues to rise and represents a major public health concern, particularly in patients co-infected with human immunodeficiency virus (HIV). The infection has a multitude of clinical presentations and is often referred to as the ‘great imitator.’ We present a rare case of an isolated presumed syphilitic optic nerve gumma and characterize it using newer imaging modalities.ObservationsA 36-year-old HIV-positive man, compliant with treatment, presented with a five da… Show more

“…Ocular syphilis can be difficult to diagnose, given its ability to masquerade as different clinical entities, such as dacryoadenitis, interstitial keratitis, or even retinal vasculitis . It can affect the optic nerve as perineuritis, optic neuritis, or papillitis, characterized by optic nerve swelling with possible peripapillary hemorrhages and, rarely, venous engorgement . Neurosyphilis often intersects with ocular syphilis, and syphilitic gummas can arise throughout the central nervous system in focal areas of syphilitic meningitis, more often in early neurosyphilis …”

“…Gummas of the optic nerve are rare . A recent report demonstrated a case of a presumed optic nerve head gumma, given its fluorescein pattern and prominent telangiectatic vessels with elevation of the lesion on optical coherence tomography .…”

“…Papillitis is more common, and is characterized by mild to moderate optic nerve head swelling, with possible peripapillary hemorrhages and rarely venous engorgement. Gummas of the optic nerve are exceptionally rare, but can cause optic neuropathy 35…”

“…Optic disc swelling may or may not be present [1], and neuroretinitis [2], perineuritis [3], and a chiasmal syndrome [4] can occur. Rarely, the optic nerve may be damaged by a gumma (non-specific granulomatous lesion) in tertiary syphilis [5]. Untreated SON results in blindness [6].…”
