The aim of this study was to calculate the battery current drain of dif ferent VVI-VVIR pulse generators, with and without automatic adjustment of stimulation output, from the same manufacturer, and obtain projections on pacemaker longevity and pacing related costs. Twenty-five patients who had received a WIR pacemaker (Microny SR+ 2425T) with Autocapture function, an algo rithm able to provide automatic adaptation of device output coupled with continuously monitored cap ture threshold, were analyzed. Calculations of current drain and projections of device longevity were based on diagnostic data retrieved from device memory and on comparisons of multiple programmed set tings of the same device (Microny SR+ 2425T with Autocapture, Low Output [output = 2 X Autocapture threshold, but not less than 2.4 V] versus Standard Output [5 V]). In addition, devices with Autocapture but with different battery capacities (0.35 Ah and 0.79 Ah, Microny and Regency, respectively) were com pared to a standard device (battery capacity = 0.78 Ah) at 2.5-V and 5-V output settings. According to a series of assumptions, Autocapture increased device longevity by 53% (Microny) and by 245% (Regency) overa 15-year follow-up with a consequent reduction in pacing related costs of 25% (Microny) and of 57% (Regency) compared to the standard settings of a VVIR device from the same manufacturer. In conclusion, pacing with constant adaptation of generator output coupled with continuous monitoring of stimulation threshold has the potential to increase generator longevity and to reduce pacing related costs as compared to a standard device from the same manufacturer. The Autocapture function controls and optimizes the amplitude of the pacing pulse and saves energy. The manufacturer recommends using a special low polarization, low threshold bipolar Pacesetter lead for the Autocapture function. The purpose of this study was to evaluate the compatibility of Autocapture with previously implanted pacing leads. The study included 15 patients (mean age 13.6 +/- 3.4 years) who needed pulse generator replacement and received the VVIR pacemaker Regency SR+ or the DDDR pacemakers Affinity DR or Integrity DR with the Autocapture function. The new pulse generators connected to previously implanted ventricular leads. At the time of implantation the pacing threshold was 1.0 +/- 0.35 V at 0.5 ms, the lead impedance was 580 +/- 80 omega, and the spontaneous R wave amplitude was 7.89 +/- 4.89 mV. The polarization signal (PS) was 3.8 +/- 3.04 mV, and evoked response (ER) was 8.15 +/- 4.57 mV at the predischarge testing. Follow-up telemetry was done at months 1, 3, 6, 12, and 18. The follow-up duration was 9.4 +/- 5 months (range 1-18 months). If the results of PS and ER measurements were acceptable for autocapture, it turned on at the 1-month visit. In six (40%) patients the results were found acceptable for autocapture function. Age, lead impedance, pacing threshold, intrinsic R wave measurement, lead age, fixation mechanism, and ER measurements were not statistically different in Autocapture suitable and not suitable groups. The main reason not to activate Autocapture had been increased PS. Any significant fluctuations were not observed in pacing threshold, lead impedance, ER, and PS during follow-up. In conclusion, previously implanted pacing leads may be compatible with the Autocapture function.

Microelectrode arrays are used for recording and stimulation in neurosciences both in vitro and in vivo. The electrodeposition of conductive polymers, such as poly(3,4‐ethylene dioxythiophene) (PEDOT), is widely adopted to improve both the in vivo recording and the charge injection limit of metallic microelectrodes. The workhorse of conductive polymers in the neurosciences is PEDOT:PSS, where PSS represents polystyrene‐sulfonate. In this paper, the counterion is the fluorinated polymer Nafion, so the composite PEDOT:Nafion is deposited onto a flexible neural microelectrode array. PEDOT:Nafion coated electrodes exhibit comparable in vivo recording capability to the reference PEDOT:PSS, providing a large signal‐to‐noise ratio in a murine animal model. Importantly, PEDOT:Nafion exhibits a minimized polarization during electrical stimulation, thereby resulting in an improved charge injection limit equal to 4.4 mC cm−2, almost 80% larger than the 2.5 mC cm−2 that is observed for PEDOT:PSS.
