Item Banks for Measuring Emotional Distress From the Patient-Reported Outcomes Measurement Information System (PROMIS®): Depression, Anxiety, and Anger

Abstract: The authors report on the development and calibration of item banks for depression, anxiety, and anger as part of the Patient-Reported Outcomes Measurement Information System (PROMIS®). Comprehensive literature searches yielded an initial bank of 1,404 items from 305 instruments. After qualitative item analysis (including focus groups and cognitive interviewing), 168 items (56 for each construct) were written in a first person, past tense format with a 7-day time frame and five response options reflecting freq… Show more

“…Directly after the visit, patients completed a sociodemographic survey (including age, sex, race, work status, marital status), the Newest Vital Sign (NVS) health literacy test [39], and three Patient-Reported Outcomes Measurement Information System-based (PROMIS) computerized adaptive questionnaires: Upper-Extremity Function [12], Pain Interference [3], and Depression [33]. The NVS health literacy test is based on a nutrition label from an ice cream container, and the overall score ranges from 0 to 6.…”

“…A researcher (ERT, not involved with patient treatment) obtained informed consent before patients saw their physician. Patients filled out their age, sex, traumatic versus discrete diagnosis, comorbidities, tobacco use, marital status, and work status in addition to the following questionnaires: the validated Patients Reported Outcome Measurement Information System (PROMIS) physical function-upper extremity [13], PRO-MIS pain interference [2], and PROMIS depression [22]. Average time necessary to complete these questionnaires was 5 ± 2 minutes (range, 2-13 minutes).…”

“…A bilingual (English and Spanish) research fellow asked all 122 participants to complete three Patient-Reported Outcomes Measurement Information System (PROMIS)-based computerized adaptive testing (CAT) questionnaires: PROMIS Pain Interference [1], PROMIS Depression [28], and PROMIS Upper-Extremity Physical Function [15]. Unlike instruments with a fixed set of items, PROMIS utilizes CAT technology based on item response theory to filter items that are redundant or do not apply to the respondent, thus enabling the administration of individually tailored questionnaires with fewer items while still maintaining adequate psychometrics [7].…”

“…Somatic symptoms such as changes in appetite and sleep disturbance are not included, as they can be explained by comorbid physical conditions [28]. Higher scores represent greater symptoms of depression.…”
