Just pretend: Participation in symbolic talk by children with histories of early corrective heart surgery

Abstract: Children with histories of early corrective heart surgery (ECHS) are at risk for language, cognitive, and motor delays. This study examined parent–child play in 30 4-year-old children with ECHS and 30 typically developing children. Children were compared on basic language measures and on proportions of symbolic and nonsymbolic talk. “…Indeed, children with CHD aged 4 and 8 years old produce less symbolic talk during playtime and fewer narrative components compared to typically developing children. 6,39,40 Although we found no significant effect of age on MBCDI Expressive Vocabulary scores, our study group seemed to exhibit decreasing performances on all MBCDI scales at 24 months, as illustrated by low average means and median scores, and slightly higher proportions of children performing under the clinical cut-offs at this age. Results also revealed increasing Bayley-III Receptive Language performances, despite no change in Expressive Language scores from 12 to 24 months of age.…”

“…At the 4-year assessment, each child also engaged in a 10-minute free play session with a parent. 14 All pairs of parent and child were provided with a standard set of materials, which included a toy house, two adult figures, two child figures, a dog, furniture, and a car. Sessions were videotaped and transcribed, and a coding system used to characterize the language of the children, and the activities in which the pair engaged.…”

“…Additional analyses of a subgroup of the cohort revealed impairments in important preliteracy skills. 37 Both strategies of vital organ support involve the use of cardiopulmonary bypass and, thus, risk of central nervous system damage from microemboli, macroemboli, and hypoperfusion. Preoperative cyanosis or hemodynamic abnormalities in the preoperative or postoperative period may have contributed to the cohort's reduced performance.…”
