Juvenile Angiofibromas: A 40-Year Surgical Experience

Abstract: A series of 22 patients with histologically confirmed angiofibroma were treated primarily by surgical removal at the University of Freiburg ENT clinic during the 40-year period, 1946–1986. These tumors were staged according to size and extension on the basis of roentgenographic and surgical findings. Specific analysis of the operative approaches and recurrence rate by stage are included. Recurrent or residual disease was handled by surgical excision, irradiation, embolization, or a combination of these methods… Show more

“…Improved surgical control rates were largely achieved by improved preoperative evaluation, better surgical support measures such as embolization, and the operators' increasing experience with microscopic and endoscopic techniques. The number of recurrences per patient in this series was 1.8, comparable to the reported rate since 1960 and independent of tumor stage and the approach used for initial tumor removal 1,2 . This was true for treatment of the initial tumor as well as for the recurrent disease.…”

In two patients, two recurrences were observed, which we were able to control surgically using an endonasal and a midface deglov- ing approach.…”

“…A role for androgens in JNA has been proposed but not proven. [1][2][3][4][5] Another influence might be the expression of angiogenic growth factors by the tumor that leads to vessel growth and subsequent tumor proliferation. This hypothesis is supported by a study conducted by Schiff and colleagues 6 in 1992 that documented immunolocalization of basic fibroblast growth factor, a strong proangiogenic cytokine, in the vessel endothelium of JNA.…”
