Juvenile papillomatosis of the breast: sonographic appearance.

Abstract: Three patients with juvenile papillomatosis of the breast are described. The lesions were apparent clinically as a palpable mass. At mammography, juvenile papillomatosis was not depicted in two patients and was seen as a poorly defined mass in one case. In all patients, high-frequency ultrasound examination showed an ill-defined, inhomogeneous mass with one or several small (up to 0.4 cm), rounded, relatively echo-free areas, which were seen mainly or exclusively near the borders of the lesion.

“…The sonographic appearance was that of an ill-defined, inhomogeneous mass with one or several small, round, echo-free areas mainly at the periphery of the lesion. 22 In summary, retrospective analysis of 56 breast masses with mixed sonographic pattern revealed that the heterogeneity of the solid portion is the most helpful criterion in the differential diagnosis for tumors classified as intracystic pattern. In tumors with multiple cystic component, the lack of a defined three-dimensional contour is the most important criterion suggesting a benign process.…”

“…4,15 Ultrasound features include poor mass definition, with internal echoes of variable strength, and one or more small, rounded, relatively echo-free areas mainly at the border of the lesion. 16 Treatment is total resection with preservation of the normal breast. The condition is a marker for families having a relatively high frequency of breast carcinoma.…”

“…Chez les patientes jeunes, cet examen est réalisé en première intention. Dans la papillomatose juvénile, on retrouve fréquemment des plages hypoéchogènes, irrégulières et contenant des kystes en grappes dans lesquels des microcalcifications peuvent être individualisées [2]. De ce fait, un cliché mammographique est souhaitable, même si celui-ci reste peu contributif du fait de la forte densité mammaire retrouvée chez ces patientes.…”
