Juvenile rheumatoid arthritis of the knee: MR evaluation with Gd-DOTA.

Abstract: Synovial hypertrophy, effusion, and articular cartilage status were evaluated with gadolinium tetraazacyclododecanetetraacetic acid (DOTA)-enhanced magnetic resonance (MR) imaging in 24 knees in 24 pediatric patients (17 female, seven male; mean age, 10 years; range, 3-18 years) with juvenile rheumatoid arthritis (JRA). T1-weighted spin-echo sequences were performed with a 0.5-T unit before and immediately after injection of Gd-DOTA (0.1 mmol/kg). Substantial enhancement of synovial proliferation was seen in 2… Show more

“…One major problem with the validation of Vsm determination by MRI lies in the identification of appropriate gold standards. A semi-automated method based on computerized counting of pixels fulfilling certain criteria can be used [16,17,18,19], volume measurements can be restricted to one or few slices per joint, or a semi-quantitative grading of synovial hypertrophy can be performed [3,20,21,22]. These methods are less time-consuming and faster and more attractive for applicability in larger clinical studies or in clinical routine.…”

“…Both features have low signal intensity on T1-weighted images and high signal intensity on T2-weighted images. The very vascular proliferative synovitis has been shown to enhance with intravenously injected gadolinium-containing compounds, although imaging must be performed immediately after injection to avoid diffusion of contrast into the joint [6]. Chronic synovitis often has low signal intensity on both T1-and T2-weighted sequences.…”

“…In addition, MR is not associated with any radiation exposure. Although standard gadopentetate-enhanced MR imaging provides a high sensitivity for the detection of arthritis, it provides only a limited specificity (17)(18)(19)(20), because the enhancement of inflammatory tissues overlaps with the enhancement patterns of normal joints and other pathologic processes (17,21,22). New FR-based gadolinium (Gd) chelates are expected to improve the sensitivity and specificity of MR imaging for the diagnosis of arthritis, similar to the radiotracers mentioned above.…”
