Keratomycosis caused byCylindrocarpon lichenicola

Abstract: We present a case of keratomycosis caused by Cylindrocarpon lichenicola in a 30-year-old immunocompetent male patient living in a rural area of Formosa Province (north-eastern Argentina). No ocular trauma is reported in his case history. There are no previous reports of infections caused by this fungus in Argentina.

“…Fusarium lichenicola is an opportunistic pathogen of humans in the tropics, causing chronic cutaneous lesions through production of keratinolytic enzymes (Mangiaterra et al 2001;Kaben et al 2006). In India, F. lichenicola was reported to cause localized cutaneous infection on the hands of an immune-compromised patient which resulted from contact with infested soil while working in the field (Champa et al 2013).…”

“…In keeping with this practical need, then, we hereby formally reinstate the original name The epidemiologic links between F. lichenicola and medically implicated isolates of F. solani are clearly suggested by a review of case literature pertaining to these fungi. F. lichenicola, for example, is most frequently medically significant as an agent of relatively aggressive keratitis subsequent to ocular trauma (1,29,31,33), just as F. solani is (18,19,26,30,59). Other well-documented cases of infection caused by F. lichenicola include disseminated infection (25) and local soft tissue infection (24) in leukemia patients and peritonitis related to chronic ambulatory peritoneal dialysis (53).…”

“…Prompt diagnosis with early institution of medication for keratomycosis caused by C. lichenicola is therefore needed to avert debilitating effects on the eye, such as were seen in this patient, or visual loss as reported in literature. 3 Moreover, appropriate access to effective antimicrobials, in this circumstance voriconazole, in low- and middle-income countries is essential while restricting inappropriate use (which drives antimicrobial resistance).…”

“…Hence, keratomycosis from plant or soil particles laden with fungal materials is not uncommon in these areas. 3 C. lichenicola is a hyaline filamentous fungus which is also called Fusarium lichenicola and has been reclassified as a member of the Fusarium solani species complex. 1 However, it rarely causes keratitis, 4 although it has been implicated in cutaneous mycosis in immunocompetent patients.…”
