Public–private collaborative efforts to address healthcare challenges in low- and middle-income countries have been the focus of digital initiatives to improve both access and quality of health services. We report the early feasibility, experience, and learnings of migrating healthcare data generated from a proprietary, privately owned cloud-based environment into an on-premises National Health Data Center (NHDC) in compliance with Kenya's data management legislation. In 2018, Medtronic LABS entered into a partnership with the Kenya Ministry of Health and other stakeholders to improve access to quality services and data availability for non-communicable diseases (diabetes and hypertension), anchored on the SPICE digital health platform. Data migration from SPICE to the NHDC necessitated the establishment of multi-stakeholder coordination structures, alignment on system configuration requirements, provisioning of on-premises servers, data replication and monitoring. The data replication process showed consistency in format and content with no evidence of data loss. The monitoring of the server uptime and availability, however, exposed overall downtime of 15% of the total time tracked between April and December 2022 caused by Internet Protocol address configuration issues, power outages, firewall rule changes, and unscheduled system maintenance. Monthly tracked downtime however reduced from a high of 28% in April 2022 to 5% in December 2022. Our early experience shows that data migration from proprietary host environments to public “one-stop-shop” national data warehouses are feasible provided investments are made in the requisite infrastructure, software and human resource capacity to ensure long-term sustainability, maintenance, and scale to match cloud-based data hosting. Further, digital health solutions developed in collaboration with non-state actors can be integrated into national data systems, saving Governments the cost and efforts of building similar tools while leveraging private sector capacity.

Hybrid cloud architectures have gained significant traction in recent years due to their ability to combine the benefits of public and private clouds. However, ensuring optimal performance in hybrid cloud environments presents unique challenges. This paper addresses the need for effective performance testing in hybrid cloud setups. It begins by discussing the key factors that impact performance, including network latency, resource allocation, workload management and data transfer. Realistic workload simulations and appropriate performance metrics are highlighted as crucial elements for accurate performance assessment. The paper then presents a comprehensive methodology for conducting performance testing in hybrid cloud environments. This methodology covers steps such as defining performance objectives, identifying workload patterns, designing test scenarios, selecting performance metrics, setting up a test environment, executing tests, analyzing results, optimizing performance and conducting iterative testing. Practical strategies, including auto-scaling mechanisms, caching, content delivery networks and efficient data synchronization techniques, are discussed to optimize performance in hybrid cloud setups. The importance of empirical studies and future research directions are emphasized to validate and enhance the proposed methodology. These include exploring advanced workload modeling techniques, adaptive testing approaches and addressing specific challenges related to hybrid cloud architectures. By adopting the methodology and recommendations presented in this paper, organizations can evaluate and optimize the performance of their hybrid cloud environments, resulting in enhanced efficiency, scalability and reliability. Ultimately, effective performance testing is crucial for unlocking the full potential of hybrid cloud deployments and achieving business success in the dynamic and evolving cloud landscape.

Nowadays, cloud computing is growing daily and has been developed as an effective and flexible paradigm in solving large-scale problems. It has been known as an Internet-based computing model in which computing and virtual resources, such as services, applications, storage, servers, and networks, are shared among numerous cloud users. Since the number of cloud users and their requests are increasing rapidly, the loads on the cloud systems may be underloaded or overloaded. These situations cause different problems, such as high response time and power consumption. To handle the mentioned problems and improve the performance of cloud servers, load balancing methods have a significant impact. Generally, a load balancing method aims to detect under-loaded and overloaded nodes and balance the load among them. In the recent decade, this problem has attracted a lot of interest among researchers, and several solutions have been proposed. Considering the important role of fault-tolerant in load balancing algorithms, there is a lack of an organized and in-depth study in this field yet. This gap prompted us to provide the current study aimed to collect and review the available papers in the field of fault tolerance load balancing methods in cloud computing. The existing algorithms are divided into two categories, namely, centralized and distributed, and reviewed based on vital qualitative parameters, such as scalability, response time, reliability, availability, throughput, and overhead. In this regard, other criteria such as the type of detected faults and adopted simulation tools are taken into account.
