Kidney Dysfunction and Left Ventricular Assist Device Support: A Comprehensive Perioperative Review

Abstract: Left ventricular assist devices (LVADs) are used increasingly as a bridge to transplantation or as destination therapy in end-stage heart failure patients who do not respond to optimal medical therapy. Many of these patients have end-organ dysfunction, including advanced kidney dysfunction, before and after LVAD implantation. Kidney dysfunction is a marker of adverse outcomes, such as increased morbidity and mortality. This review discusses kidney dysfunction and associated management strategies during the dyn… Show more

“…Cardiac and renal disease are comorbid in 30% of heart failure patients, 1 with more severe levels of renal dysfunction being found in fewer numbers of heart failure patients. 2 As these patients' cardiac disease becomes more severe heart transplantation becomes a necessary option, due to long wait times and high mortality assist devices have been utilized to increase survival to cardiac or combined organ transplantation. 1 Rates of heart failure among hospitalized patients are noted to be as high as 67%.…”

“…8,9 TAHs received FDA approval for destination therapy in 2015, and will likely become the preferred option given documented superior outcomes. 2,12 Given high burden of renal disease in patients awaiting cardiac transplantation inevitably acute kidney injury will occur postoperatively in some patients, resulting in a population of patients with an artificial device and a need for acute and/or chronic renal replacement therapy (RRT). 11 Though, acute kidney injury post-VAD and -TAH implantation often occur, presenting risks in patients with marginal or worsening renal function.…”
