Kidney Transplantation in Children With Augmentation Cystoplasty

Abstract: Based on our findings, the survival rate of the kidney is significantly lower in children with prior cystoplasty, possibly due to the higher prevalence of chronic rejection and febrile urinary tract infection in this group.

“…These patients often need some form of urinary diversion (conduit or reservoir), which is associated with a higher risk for chronic and potentially invasive infections, an increased risk for malignancies, and a less favorable outcome after renal transplantation (25,26). In addition, these patients may have various neurologic comorbid conditions and complications that could affect their long-term survival.…”

“…Bladder augmentation before renal transplantation has the benefit of preventing the neuropathic bladder-induced renal transplant damage. 37,38 Concerns have been raised that bladder augmentation may increase the technical difficulty of subsequent renal transplantation, reduce the success of peritoneal dialysis before transplantation, increase the risk of UTI, and hasten the progression to ESRD. However, recent studies have reported equivalent infection rates and no evidence of an accelerated progression to ESRD.…”
