Kidney Transplantation in COVID Pandemic—A Review of Guidelines

Abstract: The paper describes problems with the transplantation process during the COVID-19 pandemic. Transplantation procedures and programs have been impacted by COVID-19. The number of transplants has fallen noticeably. The first part of the paper points out changes in service organization, in particular donor and recipient pre-transplant and peri-transplant management. If the patients during pre-transplant evaluation need to attend face-to-face appointments, such as blood testing or other investigations, the risk of… Show more

“…There are many indications that the adoption of the vaccine will change a lot in the peritransplant procedure and allow transplantation programs to be reopened. A few basic rules and timing of vaccination against COVID-19 in transplant recipients and candidates were proposed [ 132 ]. Due to quicker lowering of post-vaccination antibodies post-transplantation, it is anticipated that, in the case of the COVID-19 vaccine, an additional shot (or schedule) will be required.…”

“…Based on accumulating evidence, the adjustment of immunosuppression was less common during the second COVID-19 wave in Spain [131]. Various transplant society recommendations usually suggest stopping antiproliferative agents or continuing the use of standard immunosuppression [132].…”

“…No patient from the ASTREGO cohort received immunotherapies or convalescent plasma, but this was administered to 2%–6% of patients in other regions. These observations show that there is no consistent approach to the management of COVID-19 in solid organ transplant recipients, but some countries have developed national COVID-19 management protocols to standardize treatments [ 24 ]. As a result, this diversity in therapeutic approaches hampers the interpretation of treatment and outcome data.…”

“…Staff in the blood collection agencies have SARS-CoV-2 nucleic acid tests (NAT) regularly, and isolation should be taken immediately once fever and other SARS-CoV-2-related symptoms appeared. In addition, donation by appointment helps to reasonably control the number of blood donors to avoid being overcrowding [39].…”
