Background and purposeEarly diagnosis is important for treatment and prognosis of obstructive sleep apnea (OSA)in children. Polysomnography (PSG) is the gold standard for the diagnosis of OSA. However, due to various reasons, such as inconvenient implementation, less equipped in primary medical institutions, etc., it is less used in children, especially in young children. This study aims to establish a new diagnostic method with imaging data of upper airway and clinical signs and symptoms.MethodsIn this retrospective study, clinical and imaging data were collected from children ≤10 years old who underwent nasopharynx CT scan(low-dose protocol)from February 2019 to June 2020,including 25 children with OSA and 105 non-OSA. The information of the upper airway (A-line; N-line; nasal gap; upper airway volume; upper and lower diameter, left and right diameter and cross-sectional area of the narrowest part of the upper airway) were measured in transaxial, coronal, and sagittal images. The diagnosis of OSA and adenoid size were given according to the guidelines and consensus of imaging experts. The information of clinical signs, symptoms, and others were obtained from medical records. According to the weight of each index on OSA, the indexes with statistical significance were screened out, then were scored and summed up. ROC analysis was performed with the sum as the test variable and OSA as the status variable to evaluate the diagnostic efficacy on OSA.ResultsThe AUC of the summed scores (ANMAH score) of upper airway morphology and clinical index for the diagnosis of OSA was 0.984 (95% CI 0.964–1.000). When sum = 7 was used as the threshold (participants with sum>7 were considered to have OSA), the Youden’s index reached its maximum at which point the sensitivity was 88.0%, the specificity was 98.1%, and the accuracy was 96.2%.ConclusionThe morphological data of the upper airway based on CT volume scan images combined with clinical indices have high diagnostic value for OSA in children; CT volume scanning plays a great guiding role in the selection of treatment scheme of OSA. It is a convenient, accurate and informative diagnostic method with a great help to improving prognosis.

This study aimed to compare the efficacy of combined treatment with kenacort and montelukast vs. surgical treatment using low temperature plasma radiofrequency ablation in children with snoring. 70 children with snoring were selected as the study subjects at our hospital and all children met the inclusion criteria. According to the different treatments received by the children, the control group (n=35) received combined treatment with kenacort and montelukast, while the observation group (n=35) underwent surgery (low temperature plasma radiofrequency ablation). The clinical treatment effects, degree of adenoid hypertrophy, symptom improvement, ventilator function, sleep quality and quality of life were compared between the two groups of children. The total effective rate of treatment in the control group was 71.43 %, while that in the observation group was 94.29 %, which was significantly higher than that in the control group (p<0.05). Before treatment, there was no significant difference in the adenoid-nasopharyngeal ratio between the two groups (p>0.05). After treatment, the adenoidnasopharyngeal ratio in the observation group was significantly lower than that in the control group (p<0.05). Before treatment, there was no significant difference in the symptom score between the two groups (p>0.05). After treatment, the symptom score in the observation group was significantly lower than that in the control group (p<0.05). Before treatment, there was no significant difference in the LAT, oxygen desaturation index 4 % and apnea-hypopnea index levels between the two groups (p>0.05). After treatment, the LAT, oxygen desaturation index 4 % and apnea-hypopnea index levels in the observation group were significantly lower than those in the control group (p<0.05). Before treatment, there was no significant difference in the Pittsburgh sleep quality index score and obstructive sleep apnea-1 score between the two groups (p>0.05). After treatment, the Pittsburgh sleep quality index score and obstructive sleep apnea-1 score in the observation group were significantly lower than those in the control group (p<0.05). Compared with combined treatment with kenacort and montelukast, low-temperature plasma radiofrequency ablation is more effective for children with snoring and surgical treatment can better improve the clinical symptoms and ventilator function of children, thereby further improving their sleep quality and quality of life.

Clinical Observation of Low-Temperature Plasma Knife Tonsil Adenoidectomy for Pediatric Snoring and Analysis of Influencing Factors

Objective. To investigate the clinical efficacy of low-temperature plasma knife tonsil adenoidectomy for pediatric snoring and to analyze the factors influencing the efficacy. Methods. 90 children with snoring who were scheduled for surgical treatment in our hospital from June 2020 to December 2021 were selected as the research objects. According to the random number table method, they were divided into control group (group C) and observation group (group O), with 45 cases in each group. The children in group C were treated with power cutting system to remove adenoids combined with conventional peeling of bilateral tonsils, while the children in group O were treated with low-temperature plasma adenoidectomy combined with bilateral tonsillectomy, and both the groups received psychological care, preoperative preparation, health guidance, postoperative posture care and close monitoring of vital signs during the perioperative period. The clinical efficacy, perioperative related indexes (including operation time, intraoperative bleeding, postoperative pain time, and hospital stay) were compared between the two groups. The apnea-hypopnea index (AHI), oxygen decrement index (ODI), longest apnea time (LAT), and lowest oxygen saturation (LSaO2) were measured before operation and 1 week after operation to evaluate the ventilatory function of the two groups. According to the curative effect, 90 children with snoring were divided into cure + significant effective group and valid + invalid group. The general data and preoperative biochemical indexes of the two groups were collected, and logistic regression model was used to analyze the related influencing factors of the curative effect. Results. The total effective rate of group O (100.00%, 45 cases) was significantly higher than that of group C (91.11%, 41 cases) (P < 0.05); the operative time, intraoperative bleeding, postoperative pain time, and hospitalization time of group O were shorter/less than those of group C; the AHI, ODI, and LAT of group O at 1 week after surgery were shorter/less than those of the control group; and LSaO2 was higher than that of group C. The differences were statistically significant (P < 0.05). Univariate analysis showed that there were significant differences in age, BMI, course of disease, preoperative AHI, preoperative LsaO2, and surgical method between cure + significant effective group and valid + invalid group (P < 0.05). Multivariate analysis showed that high BMI, high preoperative AHI, and power cutting system for adenoids combined with routine peeling of the bilateral tonsils were independent risk factors for postoperative outcome in children with obstructive sleep apnea syndrome (OSAS) (P < 0.05).
