With the popularization and application of chemical process safety management (PSM) systems in the chemical industry, the use of such systems in China has become the focus of a large volume of research. In this article, 12 elements included in the current Chinese PSM system are divided into four stages from a structural point of view. This division will be helpful in the follow‐up analysis. After a careful analysis of the evaluation results for one PSM pilot study in Nanjing, some correlations have been found between the results of the elemental evaluation. Small‐ and medium‐sized chemical plants make up a large proportion of Chinese chemical plants and many chemical accidents take place in these plants, so a simplified PSM system in practical operation need to be found. Based on a previous PSM structural analysis, the secondary elements of the PSM system can be removed. This allows researchers to avoid the traditional method of analyzing PSM elements individually. In the concrete analysis process, a theoretical analysis of the correlation degree between elements can be combined with the results of accident investigations. The simplified Chinese PSM system consists of four core elements: Process safety information, process hazard analysis, training, and emergency management. Based on the current applications of these four core elements in China, a consideration of correlations between these elements is used to make several suggestions for improving China's PSM systems. © 2017 American Institute of Chemical Engineers Process Saf Prog 36: 378–381, 2017

The increasing use and distribution of chemicals are causing serious chemical accidents such as fires, explosions, and leaks during manufacturing and handling. In most cases, all risks caused by chemicals are classified as accidents due to defects in process facilities, human errors, and multi-cause accidents. Among chemical accidents caused by human errors, accurate analysis of accidents caused by the complex action of various types of human failures is required. Based on the accident investigation reports that occurred in South Korea from 2010 to 2017, chemical accidents caused by human failure were analyzed, and the fundamental causes were derived by classifying them into human error and violation. Human failure was analyzed according to the classification criteria for human failure of health and safety executive (HSE). As a result of the analysis, several types of human failure acted in combination to be a more significant cause of chemical accidents; incorrect application of process rules and procedures, inappropriate chemical information, lack of education, and defects in the current safety regulation were analyzed as the main causes. In addition, the cause of human errors was presented through case studies of chemical accidents in South Korea.

Despite improvements in chemical safety management systems, incidents involving the release of hazardous chemicals continue to happen. In some cases, they result in the evacuation of residents. For hazardous chemical release accidents, an evacuation plan needs to be selective enough to consider both the indoor and outdoor concentrations of nearby buildings and the time in which the maximum allowable concentration may occur. In this study, a real-time risk analysis tool was developed based on the geographic information system (GIS) in order to establish the emergency response and risk communication plan for effectively assisting decision-making personnel. A selective evacuation plan was also established by a proposed assessment module considering the indoor/outdoor pollution concentration of buildings and the release duration time of chlorine gas leakage. The GIS-based simulated modules were performed based on eleven buildings of Ulsan city, located near an industrial cluster and home to a high population density. As a result of the simulated real-time risk assessment, only four buildings were affected by chlorine gas concentration according to wind direction and diffusion time. In addition, it was considered effective to establish an indoor/outdoor evacuation plan as opposed to an outdoor evacuation plan which is outside the range of the damage. Subsequently, an emergency evacuation plan was established with the concentration of a hazardous chemical according to the decision-making matrix. This study can enlighten the real-time emergency risk assessment based on GIS while effectively supporting the emergency action plans in response to the release of hazardous chemicals in clustered plants and the community.
