Intradialytic hypertension (IDH) is the most common complication of hemodialysis patients in Indonesia. It must be controlled, to maintain the patient's quality of life and prevent worsening conditions. Some factors affecting IDH include predialysis fluid overload characterized by excessive interdialytic weight gain (IDWG), low adherence to fluid restriction and increased ultrafiltration during haemodialysis. The study aims to identify contributing factors of IDH. A case-control design was used in this study. A purposive sampling technique was used to recruit 92 samples at two centres of dialysis in Semarang, which was divided into case group (n=46) and control group (n=46). Data were analyzed using the Chi-square test by calculating Odds Ratios (OR). The finding showed that excessive IDWG (p=0,000, OR=16.95, 95% CI:5,56-51,65), low fluid adherence (p=0,001, OR=4,41, 95% CI:1,82-10,68) and excessive ultrafiltration (p=0,000, OR = 29,52, 95% CI:9,23-94,46) showed significant result. However, the incidence of IDH was not correlated with sex, age and length of haemodialysis. A greater increase in interdialytic weight requires lower fluid and higher ultrafiltration factors must be controlled precisely because those are considered as the risk factors for the high incidence of IDH. Excessive ultrafiltration is the most dominant risk factor in the high incidence of IDH.

Optimization of Thirst Management on CKD Patients Undergoing Hemodialysis by Sipping Ice Cube

Excess fluid is a major problem in patients with Chronic Kidney Disease (CKD). CKD patients undergoing hemodialysis must keep limiting fluid intake during the interdialysis period so that excess fluid does not occur. Consequently limiting fluid intake arises thirst and will affect fluid restriction compliance. Management of thirst that can be done in hemodialysis patients, among others, suck ice cubes, gurgling the ripe water and gurgling with mouthwash, each of which has a different action against patients thirst. The purpose of this study was to compare the three interventions "thirst management", namely sipping ice cubes, gargling ripe water and gargling with mouthwash against patient thirst. This study was an experimental study in 27 samples of CKD patients who underwent hemodialysis at Roemani Muhammadiyah Hospital in Semarang divided into 3 groups with each group 9 samples. The results showed that the duration of holding thirst for the group sipping on ice cubes averaged 93 minutes, the gurgling group of ripe water averaged 55 minutes and the average length of time holding the thirst group who gurgling with mouthwash was 69.71 minutes. There was no significant difference in the duration of holding thirst after sucking on ice cubes and gargling ripe water, and gargling with mouthwash (ρ value 0.061). Sipping ice cubes can fight the longest thirst compared to gargling ripe water or gargling with mouthwash. The recommendations of this study are expected to intervene to sipping ice cubes, gargling ripe water and gargling with mouthwash can be used for thirst management in hemodialysis patients. Hemodialysis patients can choose interventions to reduce the thirst that is most appropriate.

Excessive fluid intake in patients with chronic kidney failure will cause complications that lead to death. This study aims to determine the relationship between fluid intake and blood pressure in patients with chronic renal failure undergoing hemodialysis at Rs. Tk III Dr. Reksodiwiryo Padang. This type of research was descriptive correlation, with a sample of 49 respondents taken by consecutive sampling technique. Data collection and research was carried out on March 15 to July 4, 2018. The method of collecting data with observation sheets and questionnaires. Univariate analysis was carried out by presenting data in the form of the mean, standard deviation, minimum, and maximum while bivariate analysis used a correlation test with Rank Spearman. The results of the univariate study of Sistole Blood Pressure from 49 patients obtained a minimum value of 0 mmHg, and the maximum value was 60 mmHg, the standard deviation was 12.69, the mean value was 16.33 mmHg. Whereas in the Diastole Blood Pressure the minimum value is-20mmHg (reduced by 20mmHg), and the maximum value is 30, the mean value is 9.18 mmHg. The fluid intake minimum value is 825.7 ml, maximum is 2216.7 ml, and mean is 882 ml. Bivariate results showed that there was a relationship between fluid intake and systolic blood pressure (p = 0.032) and diastolic blood pressure (p = 0, 013). It is hoped that collaboration between patients, families and nurses in monitoring the patient's fluid intake to prevent complications due to fluid buildup.
