Kounis syndrome: A rare case

Abstract: Allergic angina and allergic myocardial infarction are ubiquitous diseases covering a wide spectrum of mast cell activation disorders, which are associated with acute coronary syndromes and are referred to as ''Kounis Syndrome''. Here, we report a case admitted with dyspnea and mild chest heaviness secondary to bee sting, later diagnosed as Kounis syndrome.

“…Ages 40 to 70 make up the majority of those affected (68%) [9]. In recent years, similar cases were reported by Sunder et al, Pirasath et al, and Thwe et al [5,10,11]. Kadeli et al reported a case of a severe allergic response to the angiography dye, which led to an inferior wall myocardial infarction [7].…”

“…21 The severity of serious systemic toxic reactions will generally depend on the amount of venom injected, which is correlated with the number of stings. Reported systemic toxic reactions from multiple bee stings are acute kidney injury, 22 acute myocardial infarction (KS), 23 cerebrovascular disease, 24 and death.…”

“…In literature, the association of acute coronary syndrome/myocardial infarction with hymenopteran sting has been called Kounis Syndrome. [ 5 ] In a study of 152 patients of honeybee bite done by Laxmegowda et al ., bradycardia was reported in <0.7 percent patients. [ 7 ] Our case, though, had only transient bradycardia and did not require pacing.…”

“…Localized reactions include erythema, oedema, and pain whereas allergic reactions triggered by IgE, may cause urticaria, angioedema, bronchoconstriction, and anaphylactic shock. Systemic toxic reactions may cause acute kidney injury (AKI),[ 3 4 ] acute myocardial infarction (Kounis syndrome),[ 5 ] stroke,[ 6 ] peripheral neuropathy, glomerulonephritis, and even death. Mortality rates in various studies are as high as 15–25 percent.…”
