Kutane Nebenwirkungen einer Hydroxyurea-Therapie bei Polycythaemia vera

Abstract: A 68-year-old women with polycythemia vera was treated with hydroxyurea for 8 years and developed painful ulcers on her lower legs, multiple hypertrophic actinic keratoses and a squamous cell carcinoma. After discontinuing hydroxyurea therapy the leg ulcers resolved within 8 weeks. The hypertrophic actinic keratoses and squamous cell carcinoma were treated with cryotherapy and excision, respectively. Hydroxyurea induces a variety of cutaneous side effects such as painful leg ulcers and squamous cell carcinomas… Show more

“…It's supposed that, while tacking hydroxyurea, UV radiation provides an ideal environment for the formation of cSCC. In addition, HU can markedly elevate p53 levels in basal layer keratinocytes, which increases the risk of skin cancers; it also inhibits DNA synthesis and DNA repair in ultraviolet-irradiated human cells, interfering with cell replication in the basal layer of the epidermis [12] , [13] , [14] , [15] , [16] . This pathogenic mechanism could explain the appearance of the lesion on the face which is a photo exposed skin [17] .…”

“…Multiple publications show similarities to our case regarding the proposed aetiology of the leg ulcer. Consistently, dosage and duration of HU administration, as well as the location of the ulcer and the progression of the lesion: long-term use of HU (> 5 years), (1 or 1.5 mg per day), very painful perimalleolar ulcers, with no prior trauma [ 5 ] [ 11 ] [ 12 ] [ 13 ] [ 14 ]. Notwithstanding the standard wound therapy, treatment of choice in HU-induced leg ulcers is discontinuation of the drug or substitution therapy [ 8 ].…”

“…On the other hand, skin and mucosal side effects are not completely understood and usually occur following long-term therapy; cutaneous abnormalities include xerosis, ichthyosiform lesions, dark brown pigmentation of skin folds and nails, and malleolar ulcers as well as malignant tumors [810]. …”
