Introduction: The community health agent plays an essential role as a link between the community and health institutions; the effectiveness of their work is crucial in promoting health, prevention, and detection of diseases. Motivation and empowerment could affect the performance of the community agent in carrying out his work. Objective: To determine if work motivation and empowerment are factors associated with the self-efficacy of the community health agent. Methodology: Observational and cross-sectional study. The study included community health agents in the Non-Governmental Organization ADRA in Imbabura and Pichincha, Ecuador. The sample selected by the census method corresponds to 300 agents. The instruments were the Motivation at Work Scale (R-MAWS), the Psychological Empowerment Scale, and the Professional Self-efficacy Questionnaire (AU10). Descriptive statistical analyses were performed using dispersion and summary measures—a comparative analysis with chi-square. The inferential analysis was done through logistic regression where the B coefficient and the odds ratio (OR) with 95% confidence intervals (CI) were evaluated, P < .05. The theoretical and adjusted model of the structural model allowed us to corroborate the association of the variables. Results: Motivation had a positive effect on self-efficacy (OR = 10.091, CI [5.509-18.485], P < .05), empowerment had a minimal effect (OR = 0.664, CI [0.363-1.214], P > .05), which is not significant to predict the perception of self-efficacy. Conclusion: This study suggests that to improve the self-efficacy of the work of community health agents, it is necessary to intervene through strategies that keep them motivated in the exercise of their mediation work between the community and health institutions. Likewise, empowering them would contribute to self-efficacy and thus help in the fulfillment of the functions of health agents.

El objetivo de la investigación es diseñar un modelo de dirección estratégica para mejorar el plan anual de contratación pública en un hospital de Quevedo, Ecuador-2023; estableciendo los objetivos específicos son los siguientes: a) Evaluar los procesos estratégicos que contribuyan a la ejecución efectiva del plan anual de contratación pública; b) Analizar el manual de contratación pública de un Hospital y la normativa legal vigente; c) Establecer los métodos estratégicos para optimizar el plan anual de contratación pública. La investigación es de enfoque cuantitativo, tipo aplicada, nivel explicativo y diseño propositivo; se trabajó con una población muestra de 72 funcionarios de un hospital de Quevedo, utilizando la técnica de la encuesta y el instrumento es cuestionario, datos que fueron analizados estadísticamente utilizando el sistema spss, de los datos estadísticos se establece que es necesario la un modelo de dirección estratégica para mejorar del plan anual de contratación, con la finalidad cumplir con los objetivos institucionales para brindar un servicio de calidad y calidez a los usuarios, se validó la investigación obteniendo una confiablidad del alfa de Cronbach general de 0,824 de 32 elementos. Al aplicar el coeficiente Rho de Spearman podemos observar que el coeficiente de correlación entre la variable dirección estratégica y plan anual de contratación es de 0,767 siendo un nivel medio lo que se acepta la hipótesis, se concluye que es aplicable el modelo de dirección estratégica para mejorar el plan anual de contratación pública en un hospital de Quevedo.

Public health is an important symbol of national wealth and prosperity. At present, China's public health is hindered by the poor management of public hospitals, which impacts the demographic structure and socioeconomic development. Therefore, taking the implementation of public hospital reform in China as a quasi-natural experiment, this study employed the time-varying DID model and the mediating effect to evaluate the influence of public hospital reform on public health. The results were as follows: (1) Public hospital reform can significantly improve public health, and a series of robustness tests have also confirmed the effects; (2) Government's financial support is a transmission mechanism for public hospital reform to promote public health; (3) After taking control variables into consideration, the effect of public hospital reform is stronger in the western region with a poorer economy. This research provides a vital policy reference for promoting the scope of reform and improving the health of the general public.
