“…We will not discuss which surgical approach is ideal, which fractures should be managed with an open reduction and internal fixation (ORIF), or which type of osteosynthesis is most suitable biomechanically; these issues have already been addressed by other authors, and it is beyond the scope of this work. [4][5][6]8,9 Herein, we will focus entirely on the HSMA for subcondylar mandibular fractures, 10 which is illustrated step by step.…”

“…In 2006, following the development of dedicated osteosynthesis plates, Meyer et al,4 in a French study, reported a successful clinical study of condylar fractures treated using the modified submandibular approach. Following this, an English study discussed this technical variant of the conventional Risdon approach.…”

“…Meyer et al first described the surgical technique of the modified high subangulomandibular or risdon approach in 2006 [13] . The main points of this approach are: a higher incision at 01 cm from the mandibular angle edge and shorter at 04 cm, a strict ascending subcutaneous dissection preserving the marginal branch of the facial nerve, an oblique SMAS incision at 03 cm from the mandibular angle, verification of the presence of the branches of the facial nerve at the level of the aponeurosis of the masseter muscle, then section of the masseter muscle on the same oblique line exposing the posterior edge of the ramus, to then easily expose the fractures of the condyle and perform osteosynthesis with 3D plates.…”

“…Facial paralysis is the major constraint for surgeons, but the high subangulomandibular approach remains the safest [3] . This is why, in our department, it has been used for several years for low and neck condylar fractures.…”

“…Number of approaches have been described, some similar to the design [ 2 ] that is used in the current case and others including postramal (Hinds), submandibular (Risdon), transbuccal trocar and intra-oral approaches with numerous modifications [ 3 - 5 ]. But each one carries its own complications and difficulties and hence, in authors´ view, an incision design with more specialised approach is needed or at least present one more option to aesthetically designed incisions.…”
