Lactase deficiency—a comparative study of diagnostic methods

Abstract: The diagnostic value of 1-14C-lactose breath test was compared with the standard lactose tolerance test and lactase assay in jejunal biopsies in 16 control subjects, 14 patients with lactase deficiency (LD) proven by lactase assay and 20 patients with irritable bowel syndrome (IBS). 14CO2 specific activity in the 2-hr breath collection after administration of 1-14C-lactose (5 muCi) provided a satisfactory separation between the control and LD group. Values were 7.0 +/- 2.0% dose administered/mmoles 14CO2 X 10(… Show more

“…Symptoms of lactose malabsorption generally occur after ingestion of 6–12 g lactose (120–240 ml of milk) (77, 78, 79). Although high doses of lactose (≥50 g) have been used for lactose breath test (80), 25 g (equivalent of 500 ml of milk) is within the normal range of consumption and is the recommended dose.…”

“…Experi mental distension leads to increase in bowel motility which is larger in IBS than in normal control [95,96], Various biochemical abnor malities have been reported in IBS but their significance is unknown [97], There is an increased prevalenceoflBS in first-degree rel atives [91]. Lactose malabsorption causes similar symptoms but is an unrelated disorder [58,98].…”

“…The direct determination of the lactase activity of the small intestinal mucosa has been proposed as the reference method with which all indirect diagnostic methods should be compared [28][29][30][31]. A low jejunal lactase activity is always present in hypolactasia.…”
