Langerhans cell hyperplasia in scabies: a mimic of Langerhans cell histiocytosis

Abstract: Because of the prominent CD1a+/S100+ component, scabies can mimic Langerhans cell histiocytosis. This finding should be considered in conjunction with scattered CD30+ cells and clinical features to avoid misdiagnosis.

“…This may indicate hypothetically that LCH arises in some conditions in a background of Langerhans cell hyperplasia or some of them are not even LCH. Similarly, some scabies infestations are demonstrated to be associated with Langerhans cell hyperplasia, which is totally cleared following the treatment of underlying pathology that is believed to cause an antigenic stimulus for epidermal Langerhans cells [14,15].…”

“…The cause of LCH is still unknown [6], although there are some theories which have grown from the research conducted over the past few years. However, there is evidence that Langerhans cell proliferations represent a spectrum of disease that ranges from reactive to neoplastic phenomenon [12][13][14][15].…”

“…316 B-lymphoproliferative and Tlymphoproliferative disorders, as well as benign inflammatory skin conditions, including scabetic nodules, can demonstrate numerous Langerhans cells, requiring correlation with the clinicopathologic setting. 317,318 CUTANEOUS METASTASES Adenocarcinomas can metastasize to the skin, and rarely, that can be the first manifestation of the primary diagnosis. Identification of the site of origin has profound prognostic and therapeutic effect.…”
