Lansoprazole, clarithromycin and metronidazole for seven days in Helicobacter pylori infection

Abstract: A 1-week course of lansoprazole 30 mg o.m., clarithromycin 250 mg b.d. and metronidazole 400 mg b.d. eradicates H. pylori in up to 86% of patients. It is of proven benefit in patients with pre-treatment metronidazole-resistant strains of H. pylori.

“…Low-dose triple therapy with an antisecretory drug (7 days) Bouchard 49 Lerang 62 Thijs63 Miyaji 64 Harris 65 Moayyedi 66 Lind 67 for the discrepancies observed between studies with regard to the clinical relevance of metronidazole resistance. As mentioned earlier, it is possible that the E-test may overestimate the true rate of metronidazole resistance 48 . Moreover, correlations between different susceptibility testing methods have yielded suboptimal results for metronidazole, with only nine of 16 E-test MIC values being in the range of ± 1 dilution of those obtained by the agar dilution 56.…”

“…Some studies suggest that triple therapy consisting of a proton pump inhibitor (PPI), CLR, and a nitroimidazole is effective in eradicating H. pylori infection in patients with MET-resistant strains [6][7][8][9], whereas others show that MET resistance significantly decreases efficacy [10][11][12]. This discrepancy may be due to the lack of a standard method for determination of susceptibility.…”

“…Alternative regimens should be used in such patients. As a regimen without amoxicillin, triple therapy with a proton pump inhibitor (PPI), clarithromycin and metronidazole (4) has been reported. However, the eradication rate associated with this triple therapy is low in patients infected with clarithromycinresistant strains of H. pylori (5).…”
