Laparoscopic assisted pancreaticoduodenectomy: an important link in the process of transition from open to total laparoscopic pancreaticoduodenectomy

Abstract: Background: The safety of total laparoscopic pancreaticoduodenectomy still remains controversial. Laparoscopic assisted pancreaticoduodenectomy (LAPD) may be an alternative selection. The purpose of the present study is to compare a consecutive cohort of LAPD and open pancreaticoduodenectomy (OPD) from a single surgeon. Methods: A comparison was conducted between LAPD and OPD from January 2013 to December 2018. Perioperative outcomes and short-term oncological results were compared. Univariate and multivariabl… Show more

“…In this circumstance, LAPD is an alternative procedure before performing total laparoscopic PD. Tian et al 3 reported the study of LAPD compared to open PD showed longer operative time in the LAPD group but no difference in clinically relevant POPF, major complication, intraoperative blood transfusion, and R0 resection rate. The authors suggested that LAPD be used as a bridge between open and total laparoscopic PD.…”

“…2 Laparoscopic-assisted PD (LAPD) was reported as a linking procedure to total laparoscopic PD with comparable perioperative morbidity, mortality, and oncological outcomes to open PD. 3 We present the first successful LAPD at Songklanagarind Hospital, tertiary-care center, Thailand, the procedure was performed in July 2018.…”

“…Similar results were also reported by Tan et al [76] and Mendoza et al [77] , who showed no differences in oncological and perioperative outcomes between open PD and LAPD. Tian et al [68] reported a lower estimated blood loss and shorter time to first flatus and Wang et al [67] described again a lower intraoperative blood loss and a shorter LOS. Additionally, a lower rate of anastomosis related complications has been reported compared to totally laparoscopic PD performed by experienced pancreatic surgeons at the beginning of their learning curve [78] .…”

“…In LAS, the preparation and part of the demolition phase of the surgical operation is managed via laparoscopy, while the reconstruction part is performed out of the body via a small laparotomic incision [66] . With this approach, we are able to take advantage of the improved vision of secluded spaces given by the laparoscopy, sparing a large incision to the patient and granting a faster postoperative recovery, while assuring an adequate anastomosis technique and hemostasis through a small incision that can also be used for the retrieval of the resected specimen [67,68] . Several authors, in the initial phase of approaching minimally invasive PD, used a laparoscopic-assisted PD (LAPD) approach and reported their case series, proposing the feasibility of LAPD [69][70][71][72][73][74] .…”

“…The incidence of comorbidity in our study was equivalent to others performing left approach, and was not higher than those performing other SMA-rst approach. Other publication show higher open conversion rate, such as Treeoongckaruna (17.7%), Boggi (9.1%), Feng Tian (16.7%) [15][16][17] . The median total blood loss was 100 mL.…”
