Laparoscopic cholecystectomy in children with chronic hemolytic anemia

Abstract: Elective LC should be the gold standard in children with CHA and asymptomatic cholelithiasis in order to prevent the potential complications of cholecystitis and choledocholithiasis, which lead to major risks, discomfort, and longer hospital stay.

“…Proper preoperative work up, knowledge of possible complications and adequate training makes this operation a safe procedure with favorable result and lesser complications 41 . Curro et al, recommend elective early laparoscopic cholecystectomy in children with chronic hemolytic anemia and asymptomatic cholelithiasis in order to prevent the potential complications of cholecystitis and choledocholithiasis which lead to major risks, discomfort and longer hospital stay 42 . The British guidelines for management of hereditary spherocytosis recommended that; in children undergoing splenectomy, the gall bladder should be removed concomitantly if there are symptomatic gallstones.…”

“…However in another study of laparoscopic cholecystectomy, the mean age was 43.7 years with a female to male ratio of 4.5:1.7. In another clinical study of 281 cases of laparoscopic cholecystectomy, there were 140 men and 141 women with a mean age of 56.9 years (range 23-89 years) 9,10 . In this study, among the total 172 patients of laparoscopic cholecystectomy, in case of 69.2% patients, laparoscopic cholecystectomy was done for chronic cholecystitis and in 18.6% cases, it was performed for acute cholecystitis.…”

“…P values (using the Chi square test) are more than >0.05 here, hence suggest insignificant results statistically. In a research study, the incidence rates of chronic and acute cholecystitis for which majority (89.7%) of laparoscopic cholecystectomy was done were 77.5% and 12.2% respectively 10 .…”

“…Although ultrasound imaging is sufficient for the diagnosis of gallbladder stones, scintigraphy (for ejection fraction) and magnetic resonance cholangiopancreatography have been claimed to be necessary to display bile ducts and the flow of the bile in patients having episodes of cholangitis or biliary dyskinesia [1,7,8]. In our series, the diagnosis was made using ultrasonography for all patients.…”
