Laparoscopic conversion of vertical banded gastroplasty to a Roux-en-Y gastric bypass

Abstract: The vertical banded gastroplasty was the mainstay of bariatric surgery for over a decade. Though this procedure is now rarely performed many of these patients will present with failure or maladaptive eating and its sequelae. Some of these patients who demonstrate the motivation for lifestyle modification as well as many of these with complications will be candidates for revisional surgery. This article reviews the technical challenges in performing these revisions using minimally invasive techniques. In additi… Show more

“…Although this series is small, the ease of revision after TOGA contrasts with difficulties that have been observed during revision of laparoscopic adjustable gastric banding (LAGB) or vertical banded gastroplasty (VBG), which are reported as technically demanding and associated with higher operative morbidity and mortality compared with primary bariatric surgery [4][5][6][7][8][9][10]. The current observation suggests that performing LRYGBP after TOGA may be less technically challenging than after VBG or LAGB, and consequently potentially safer.…”

“…Laparoscopic RYGBP, LAGB, LSG, and laparoscopic duodenal switch compromise the majority of bariatric procedures performed currently [1,2]. As a result, the number of RYGBP-related revisions outnumbers the conversional and reversal procedures.…”

“…Procedures such as jejunoileal bypass, nonadjustable gastric banding, and vertical banded gastroplasty (VBG) are no longer accepted as options for primary bariatric surgery [1,2]. They have largely been replaced by Roux-en-Y gastric bypass (RYGBP), laparoscopic adjustable gastric banding (LAGB), biliopancreatic diversion with duodenal switch, and laparoscopic sleeve gastrectomy (LSG) [2].…”

“…The details for RYGB have been described previously in detail [8,20,21]. Briefly, in open procedures, a midline incision was used and adhesions were divided as necessary.…”
