Laparoscopic histological mapping for the determination of the length of aganglionic segment in children with Hirschsprung disease

Abstract: Background. Modern approach to the surgical treatment of Hirschsprung's disease (HD) consists in the earliest possible repair and reduction of the number of surgical interventions. Primary one-stage transanal endorectal pull-through (TEPT) technique requires preoperative determination of the length of aganglionic segment. The efficacy of the standard method -contrast enema -is questionable in patients with a poorly defined transitional zone.Objectives. To present the proposed laparoscopic method for the manage… Show more

“…Minimally invasive colonic mapping in HSCR has been rst reported by Mazziotti and Carvalho in 2001 [15,16] with good results but in a relatively small series of patients. Later on, in 2021, Bogusz et al reported similar results and outcome in a slightly larger series [17]. These authors reported safety and reliability of a minimally invasive approach for the diagnosis of intestinal innervative issues in HSCR.…”

“…This is particularly important as approximately 10% of neonates show mismatch between radiographic transition zone and level of aganglionosis ( 93 ). As a result, for any case with confirmed or suspected non-classic segment HD, the use of laparoscopic assistance seems to be required ( 94 ).…”
