Laparoscopic Modified Taylor Procedure in the Treatment of Duodenal Ulcer: Technique and Outcome after 5-Year Follow-Up

Abstract: Background: Recently, gastric stapling with posterior truncal vagotomy has been performed by laparoscopic surgery, as an alternative to highly selective vagotomy (HSV) and the Taylor procedure for the treatment of chronic duodenal ulcer. Aim: To investigate, after a mean 5-year follow-up, the effect of the stapling-modified laparoscopic Taylor procedure, on gastric secretion, emptying and reflux as well as clinical parameters and recurrence rates in patients treated for duodenal ulcer. Methods: 16 patients, ag… Show more

“…Urbano et al in their study show that the treatment of choice for perforated ulcer is Taylor conservative procedure and laparoscopic drainage of the abdominal cavity [16]. Petrakis et al demonstrated that the modified laparoscopic stapling Taylor procedure (GomezFerrer method) allows a more rapid, technically easier and radical performance of the operation with excellent long term results [17,18]. Peritoneal lavage and drainage, constituting the most important part of laparoscopic procedure for perforated ulcer repair, should be very thorough.…”

“…To shorten the operation time, posterior truncal vagotomy was developed along with seromyotomy and the Hill-Barker procedure. [11][12][13][14] Most modified procedures comprise posterior truncal vagotomy and anterior HSV to decrease the operation time. In East Asia, laparoscopic gastrectomy with lymph node detection has become the standard operation for early gastric cancer surgery.…”

“…To shorten the operation time, posterior truncal vagotomy was developed along with seromyotomy and the Hill-Barker procedure. [12][13][14] The aim of the present study was to compare the clinical and operative results between laparoscopic primary repair (LPR) alone or with HSV in patients with duodenal ulcer perforation. To the best of our knowledge, this is the first report of LPR with concurrent anterior and posterior HSV procedures in patients with duodenal ulcer perforation.…”
