Laparoscopic Renal Biopsy via Retroperitoneal Approach

Abstract: Renal biopsy was performed in 17 patients using the balloon technique of endoscopy of the retroperitoneum. There was good exposure of the kidney and a sufficient amount of renal tissue was obtained in all patients with minimum postoperative morbidity. The preliminary results show that this method could become a safe and reliable alternative to open surgical biopsy.

“…A laparoscopic (through a retro-or transperitoneal approach) or open kidney biopsy may be the best option in selected circumstances, such as morbid obesity, solitary kidney, coagulopathy, failed PRB, polycystic kidney disease with rapidly progressive GN, high location of the kidney, and/or poor visualization with imaging (26)(27)(28)(29). These approaches have the theoretical advantage of direct visualization and application of hemostatic materials (such as absorbable gelatin and oxidized cellulose) to the biopsy sites, but no studies have been performed to show improved complication rates.…”

“…Renal biopsy was reinforced after the report by Iversen & Braun showing PNRB as a safe, easy and less morbid method (15 Although PNRB is the standard procedure to obtain renal tissue samples nowadays, it is not indicated in different situations such as noncollaborative patients, failure to obtain samples for pathology tests, solitary kidney, morbid obesity and hematological disorders (1-4). In these cases, some authors suggest laparoscopic approach as an alternative to ORB (7)(8)(9)(10)(11)(12)16).…”

“…As an alternative to ORB in selected cases, some authors propose transperitoneal (7) or retroperitoneal laparoscopic approach (8)(9)(10)(11)(12). Laparoscopy allows potential advantages such as less postoperative pain, better cosmetic results, short hospitalization and convalescence (13,14).…”

“…In these patients other options for biopsy, e.g. open renal [2,3], laparoscopic renal [4][5][6][7], endovascular [8][9][10], CT-guided and ureteroscopic [11] could be used; each has specific advantages and limitations.…”
