Laparoscopic Right Donor Nephrectomy: A Large Single-Center Experience

Abstract: This large single-center experience demonstrates that laparoscopic right donor nephrectomy performed without hand-assist devices is safe and yields kidneys with excellent function. The authors conclude that selection of the appropriate kidney for donation using this approach can be based on the same criteria that have traditionally governed open donor nephrectomy.

“…However, with increasing experience and progress of technology like Ultracision Ò and Ligasure Ò devices large series from highvolume transplant centres have overcome these difficulties and demonstrated ecellent outcomes for donors and recipients after laparoscopic right donor nephrectomy [16,17,20]. The prediction has been made that laparoscopic live donor nephrectomy will be the new gold standard in the near future [10].…”

“…Placement of a laparoscopic Satinsky clamp, division of renal vein by a Metzenbaum scissor followed by an intracorporal running suture of the vena cava is another way to gain maximal renal vein length [9]. In their large single centre experience Posselt et al [20] used a 2.5 mm vascular TA stapler, leaving staples on the cava side only, before dividing the vessel. In our setting we put some traction to the loop which we placed around the renal vein to maximize the vessel length and we divided the renal vein with the application of a vascular roticulator Endo-Gia.…”
