Laparoscopic subtotal cholecystectomy for difficult cases of acute cholecystitis: a simple technique using barbed sutures

Abstract: Background Laparoscopic cholecystectomy (LC) for difficult acute cholecystitis (AC) cases bears a high risk of vasculobiliary injuries (VBI). The Tokyo Guidelines 2018 (TG18) recommend the use of bailout procedures and subtotal cholecystectomy to prevent VBI. Performing a safe LC is challenging, even when followed by an accurate pre-surgical assessment. Laparoscopic cholecystectomy (LSC) requires advanced skills, and there is a risk of recurrence of cancer and/or gallbladder stones (GBS) in the remnant gallbla… Show more

“…Various methods have been reported to prevent bile leakage during laparoscopic subtotal cholecystectomy [10][11][12]. Fujiwara et al described a technique where the neck of the gallbladder was closed using continuous sutures with a barbed suture [10]. Kato et al reported a case in which omentum was sutured to the gallbladder stump to prevent bile leakage [11].…”

“…Appropriate treatment of the cystic duct or gallbladder stump can reduce the risk of postoperative bile leakage. Fujiwara et al 9 reported the usefulness of barbed sutures for the stump. A continuous suture with barbed suture can adjust not only the suture pitch and bite, but also the tension according to the thickness, strength and flexibility of the edematous tissue.…”
