Laparoscopic surgery for complex and recurrent Crohn’s disease

Abstract: Crohn’s disease (CD) is a chronic inflammatory disease of digestive tract. Approximately 70% of patients with CD require surgical intervention within 10 years of their initial diagnosis, despite advanced medical treatment alternatives including biologics, immune suppressive drugs and steroids. Refractory to medical treatment in CD patients is the common indication for surgery. Unfortunately, surgery cannot cure the disease. Minimally invasive treatment modalities can be suitable for CD patients due to the beni… Show more

“…[15]. Several studies have been published since 2010, many of which have focused on the feasibility of a laparoscopic approach in these patients with recurrent CD [16][17][18][27][28][29][30][31]. These studies were included in a recent meta-analysis, which did not highlight any significant increase in the postoperative complication rate in the recurrent group (odds ratio = 1.41; 95% CI 0.86-2.34; P = 0.18) [32].…”

“…A limited visual field and poor exposure are often caused by thickened inflammatory mesenteric blood vessels and a dilated bowel. These difficulties are not only a great challenge for open surgery, but also for laparoscopic surgery[ 14 , 15 ].…”
