Laparoscopic surgery

Abstract: Patients who undergo several in-vitro fertilization (IVF) treatment cycles and fail to conceive present a frustrating problem to the clinician. When 1 cycle of IVF treatment fails, should we offer the couples to choose additional cycle of IVF instead of evaluation of the potential peritoneal factor? In cases of otherwise unexplained infertility, the investigation cannot be considered to be complete until laparoscopy has been performed. The aim of the study is to investigate the fertility outcome of laparoscopi… Show more

“…Independently from the ovarian reserve, in presence of hydrosalpinxes (often related to a PID history) a diagnostic and operative laparoscopy should be proposed before IVF (63)(64)(65)(66). In case of dysmenorrhea/dyspareunia or others signs suggesting for mild endometriosis or IVF refusal (67,68), a diagnostic laparoscopy within 3 months could be proposed, because undiagnosed or subtle pelvic abnormalities may be a significant cause of IVF failure (69).…”

“…Correcting these pathologies at laparoscopy resulted in a spontaneous cumulative pregnancy rate of 50-60 %. In a smaller study including 45 patients with unexplained infertility and failed IVF (Yu et al, 2019) the reported presence of endometriosis, tubal pathology and adhesions was respectively 57.7 %, 31.1% and 33.3% of the patients. In the study of Kanda et al (2006) the diagnosis of unexplained infertility before and after laparoscopy decreased from 48.9 % to 19.5%.…”

“…Although EP could be treated conservatively, it might make sense to perform an active surgical evaluation of patients after embryo transfer. It was reported that laparoscopic surgery to assess pelvic conditions after IVF-ET failure helped increase the rate of pregnancy in the IVF-ET cycle [ 10 ].…”
