Laparoscopic versus open right hemicolectomy: a comparison of short-term outcomes

Abstract: Laparoscopic right hemicolectomies are as feasible and safe as the open technique. They confer improved cosmesis with smaller incisions but at the expense of longer operating time.

“…Similarly, a laparoscopic approach has previously been demonstrated to decrease the wound infection rate, expedite the average length of hospitalization, and significantly reduce postoperative morbidity [40,41]. While some studies are similar to ours and have shown a decrease in immediate mortality with the laparoscopic approach [40,42], most studies and recent meta-analyses do not [43][44][45]. Our data most likely have some selection bias as only 16 (6.8%) of the emergent/urgent colectomies were performed laparoscopically.…”

“…A recent meta-analysis of 2946 patients showed no difference in the quality of life up to 3 mo after surgery when laparoscopy was compared with open surgery for colorectal indications [19]. Studies have also shown that there is no difference between the rate of postoperative complications when open right hemicolectomy is compared with the laparoscopic approach [20][21][22]. In this study, there was one grade IV complication (anastomotic leak needing ICU management) and one grade V complication (death due to atrial fibrillation, pleural effusion, and subsequent deterioration 3 wk after hemicolectomy for metastatic colon cancer) in the OpERAS group.…”

“…Lohsiriwat et al. [14] and Tan et al [17] compared laparoscopy to transverse laparotomy in right hemicolectomies. They could not find a significant difference in parenteral analgesia requirement, bowel recovery, dietary improvements, length of hospital stay, and 30-day morbidity, implying there is no shortterm postoperative benefit of laparoscopy over a transverse incision.…”
