Laparoscopy-Assisted Transgastric Endoscopic Retrograde Cholangiopancreatography (ERCP) After Roux-en-Y Gastric Bypass: Technical Features

Abstract: In patients with a past history of Roux-en-Y gastric bypass (RYGB), the present technique allows us a standardized, safe, and reproducible access to the major papilla and the biliary tree using a transgastric access. This will lead to simplify the procedure and reduce the risk of peritoneal contamination.

“…In the presence of disorders in common hepatic ducts, it is almost impossible to perform endoscopic retrograde cholangiopancreatography (ERCP) following II-DSG. Conversely, in RYGB, it is possible to perform hand-assisted ERCP through the remnant gastric pouch [ 17 ], which is an advantage of L-RYGB with ileal transposition over II-DSG.…”

“…In this setting, some authors propose the direct insertion of the 15 mm-trocar into the excluded stomach. This will allow an easy and direct passage of the duodenoscope into the gastric remnant and may reduce the peritoneal contamination (9). On the other hand, mucosal lacerations have been described (6).…”

“…Thus, various alternative therapeutic options to reach the common bile duct (CBD) have been proposed: a) balloon enteroscopy-assisted ERCP (3); b) percutaneous transhepatic cholangiography procedure (4); c) EUS-directed transgastric ERCP (5); and d) laparoscopy-assisted transgastric ERCP (6)(7)(8)(9)(10).…”
