Laryngeal chondrosarcoma: a report of five cases

Abstract: Laryngeal chondrosarcoma is a rare tumor that is known for its indolent course and its tendency to ultimate recurrence. According to the actually adopted classification, 95% of the reported cases are of a low-grade type. A consensus recognizing conservative surgery as the most reasonable treatment for these lesions has almost been reached. However, fear of jeopardizing the patency of the laryngeal airway as a result of a wide cricoid excision and also the fear of repeated recurrences could still push some surg… Show more

“…On the other hand, ER for cricoid LCS should be attempted only in selected cases, such as in presence of a tracheotomy performed in a suboptimal position (low in the cervical trachea) or in patients with severely compromised general conditions. Previous reports have also described ER as a procedure to deal with recurrences, until functional compromise and the inability to reconstruct an adequate airway dictate the necessity for TL …”

“…Laryngeal CS typically develops in the elderly, with a moderate male predominance, and is estimated to be seven times more common in Caucasians . Age at diagnosis usually ranges from 40 to 80 years, with a peak coinciding with the period of hyaline cartilage ossification . The vast majority of cases (around 80%) are situated in the cricoid cartilage, with a special predilection for the paramedian inner surface of the posterior plate .…”

“…33,34 Recent reports have described endoscopic CO 2 laser therapy as a procedure to deal with recurrences, or even primary lesions, [33][34][35] depending upon the extent of the tumor, until functional compromise and the inability to reconstruct an adequate airway dictate the necessity for total laryngectomy. But only 1 patient in our series was disease free after 4 repeated endoscopic surgeries.…”

“…Total laryngectomy is indicated in cases of high-grade malignancy, when total exeresis of the lesion is not feasible, or in recurrent lesions 5,11 . Chondrosarcomas are little radiosensitive and radiotherapy is reserved for cases of recurrence, in extensive forms, for high grades or irresectable lesions 13 . Chemotherapy is the palliative management for aggressive tumors with local invasion.…”
