Laryngeal Manifestations of Rheumatoid Arthritis

Abstract: Rheumatoid arthritis is a destructive autoimmune disease that affects 3% of the adult population. It is characterized by the formation of both articular and extra-articular lesions with predilection for small joints. There are ubiquitous reports on the head and neck manifestations of RA with emphasis on the larynx. The laryngeal presenting features of this systemic disease may mimic a plethora of medical conditions, inflammatory and neoplastic. The main phonatory and respiratory symptoms are often subtle and m… Show more

“…9) Other symptoms may include laryngeal mucosal edema, myositis of the intrinsic laryngeal muscles, inflammatory masses or rheumatoid nodules, and bamboo nodes. 6) The clinical presentation of laryngeal rheumatoid nodules vary from being asymptomatic to causing life-threatening respiratory symptoms and include sore throat, cough, foreign body sensation, odynophagia, hoarseness, dysphonia, change in voice quality, and respiratory distress. 5,6) The previous papers published in Korea have reported only two cases of vocal cord paralysis with cricoarytenoid joint fixation caused by rheumatoid arthritis.…”

“…Less commonly we can see crico-arytenoid subluxation (39.9%), decrease in the cricoarytenoid joint space, erosion and ankylosis of the cricoarytenoid joint, and soft tissue changes, such as narrowing around the glottis and pyriform fossae (Fig. 5) [46,47]. MRI scans can also be used [3].…”

“…Bronchiolitis obliterans, a form of severe constrictive bronchiolitis, occurs rarely in young patients with RA and constitutes an untreatable condition leading to severe respiratory failure and premature death [59] . Rarely also the upper airways may be involved in RA under different forms such as rheumatoid nodules of the vocal cords, vasa nervorum vasculitis of the recurrent laryngeal and/or vagus nerves, and cricoarytenoid arthritis [60] . Occasionally, emergency measures may become necessary to ensure the patency of the airways [61] .…”
