Background and objectives:Tracheal intubation associated with airway operations can cause complications such as laryngospasm, bronchospasm and periods of reduced oxygen saturation. Such complications are frequently reported during adenotonsillectomies, a procedure that by nature increases the incidence of airway complications. The objective of this study was to compare the occurrence of respiratory problems during adenotonsillectomies while using either a disposable laryngeal mask airway (LMA) or an endotracheal tube (TT).

A 2 year-old boy presented with acute upper airway obstruction following a 15-month history of noisy breathing and hoarseness. An urgent laryngotracheal bronchoscopy was performed following inhalational induction of anesthesia. Using a fiberoptic bronchoscope, visualization of the larynx through a laryngeal mask airway revealed a flat plastic Christmas tree embedded within granulomatous cords causing almost complete obstruction and requiring tracheostomy prior to extraction. Twelve days later, the tracheostomy was successfully decannulated with the child's voice beginning to normalize. The family remembered the decoration from Christmas celebrations 2 years prior and recalled a coughing episode that predated the onset of hoarseness.

Foreign body aspiration is a real emergency for chidlren. Mortality and morbidity rates are directly correlated with clinical symptoms present during admission. The patients may present with a range of symptoms from cardiopulmonary arrest to mild respiratory symptoms without an apparent history of foreign body aspiration. This may lead to delays in diagnosis for several weeks or months which may further worsen lung problems. Chest X-ray or high-resolution computed tomography in the diagnosis phase could be beneficial. Bronchoscopy is used for both confirming the diagnosis and removing the obstruction in airways. Although there is no consensus on which method should be used during these procedures, pharmacological and technical advances in the field of anesthesia has lead to significant decrease in morbidity and mortality. The ventilation and anesthesia methods that would be applied largely depend on the cooperation and compliance between bronchoscopy user and anesthetist. In this article, anesthetic management used to remove foreign bodies in children to remove airway obstructions has been briefly reviewed.
