Lasers and the treatment of periodontitis: the essence and the noise

Abstract: The dental literature contains 25 years of accumulated reports and clinical studies addressing the utility of lasers in the treatment of periodontitis, both as a monotherapy or as an adjunct to surgical and nonsurgical therapy. Yet, the evidence from the 118 human clinical studies cited in this narrative review remains conflicted and insufficient to suggest that integration of a laser in a periodontal treatment protocol will provide antimicrobial and healing outcomes superior to those achieved by traditional t… Show more

“…Laser therapy provided minimally invasive soft tissue and biostimulatory effects on tissue regeneration. 101,102 Laser therapy significantly improved clinical parameters, including the plaque index, gingival index, probing depth, clinical attachment level and BOP. 89 The biochemical parameters IL-1β and TNFα were reduced as well.…”

“…It is true that some adjunct therapies for periodontal disease in humans are available, including antibiotics and low-level laser treatment for antimicrobial and antibiofilm approaches, respectively (50,51). However, long-term use of antibiotics is known to develop antibiotic-resistant bacteria (52), while the laser may cause tissue damage (53).…”

“…47 The evidence from a recent review on the use of lasers in the treatment of periodontitis remains conflicted and insufficient to suggest that the integration of a laser in a periodontal treatment protocol will provide antimicrobial and healing outcomes superior to those achieved by traditional therapy. 51 Further reading For further resources and reading material concerning periodontal care in general practice, see Box 1. The associations between periodontitis and general health should also be discussed to improve motivation.…”
