Late complications of electrotherapy — a clinical analysis of indications for transvenous removal of endocardial leads: a single centre experience

Abstract: Clinical manifestations of electrotherapy complications in the study group varied and included a relatively small number of infectious complications (24%) and a relatively large number of late myocardial perforations (14%). Efficacy and safety of the procedures were very high.

“…Our study has implications for both lead manufacturers and lead implanters. Smaller diameter leads have, as a group, resulted in greater risk of significant complications such as perforation or lead failure, with a decrease in venous stenosis as a potential benefit. With that in mind, implanters need to consider whether an individual patient would benefit from a larger lead with proven durability at initial implant.…”

“…The problem is the lack of consistency in the time that would be the dividing line. In the literature, the limit for early complications is 1 month after the procedure [ 3 ] but also 1 year as in the case of infectious complications associated with artificial valves [ 5 , 6 ]. The second criterion mentioned above is that the lead is usually removed by simple traction by this time, and after more than 1 year, due to the healing and ingrowing process, it is often necessary to use more complex equipment to perform transvenous lead extraction (TLE) [ 7 ].…”

“…Sometimes, it leads to complete or partial emergence of the device. In the past, a popular form of therapy was local surgical treatment of the pocket, when all elements of the pacing system were left in the patient’s body, which led to the suppression of the inflammatory process and a delay in proper management – transvenous lead extraction [ 3 , 6 ].…”
