Late toxicity of image-guided hypofractionated radiotherapy for prostate: non-randomized comparison with conventional fractionation

Abstract: A small increase in mild late toxicity, but not statistically significant increase in severe late toxicity in the hypo-IGRT group when compared with conventional non-IGRT group was observed. Our study confirmed that IGRT allows for safe moderate hypofractionation, offering a shorter overall treatment time, a good impact in terms of RFS and providing potentially more economic health care.

“…RT is a common therapeutic choice in prostate cancer, it can be performed by external beam RT or brachytherapy [82][83][84][85]. RT may induce physiological changes that must be correctly interpreted given that they can hinder tumor detection as well as mimic tumor recurrence.…”

“…After further exclusion, 26 articles were eligible for inclusion, but 6 of them were excluded due to the inappropriate form of outcomes [17] , [37] , [38] , [39] , [40] , [41] and 2 were excluded because of the same cohort of patients [42] , [43] . Ultimately, 18 studies that met the eligibility criteria were finally selected, including 3 RCTs, [19] , [23] , [44] 4 PCSs [45] , [46] , [47] , [48] and 11 RCSs [18] , [21] , [22] , [49] , [50] , [51] , [52] , [53] , [54] , [55] , [56] . “…The upper limit for the prescribed dose for DIL was considered 70 Gy in 27 fractions for high risk DILs. This hypofractionated dose escalation was used and evaluated in many studies (35)(36)(37)(38)(39)(40)(41)(42). According to a study by Onjukka et al (43), this hypofractionated dose was equivalent to 86 Gy in 37 sessions (used in the study of Uzan et al (44).…”
