Latent (slowly progressing) autoimmune diabetes in adults

Abstract: About 10% of patients with the clinical presentation of type 2 diabetes suffer from an autoimmune form of diabetes associated with a rapid decline of residual beta-cell mass and subsequent development of insulin dependency. In this condition, called latent autoimmune diabetes in adults (LADA), there are clinical and metabolic features intermediate between type 1 and type 2 diabetes. Recent studies provide novel information on the immune markers associated with progressive beta-cell loss in LADA patients. Howev… Show more

“…No treatment guidelines for LADA have been published; therefore patients are mostly treated as affected by type 2 diabetes [ 3 ]. Most patients are treated with diet and oral hypoglycemic agents until the secondary failure to treatment with oral hypoglycemic agents results in the need to switch to insulin therapy [ 12 ]. Approximately 25% of LADA patients have serological evidence of autoimmune thyroiditis, but there are no reports on the frequency of clinical thyroid disease [ 10 , 13 ].…”

“…Latent Autoimmune Diabetes of Adults (LADA) is a form of adult autoimmune-mediated diabetes that phenotypically and genetically is intermediate between T1DM and T2DM [16,17]. Its diagnostic criteria are still controversial.…”

“…LADA should be diagnosed by the internists on the bariatric team by the presence of low plasma insulin, very low fasting and meal-stimulated C-peptide, and antibodies to glutamic acid decarboxylate, insulin, and/or islet cells [25][26][27][28][29][30]. These patients may initially respond to oral antidiabetic medication or GLP-1 while there are still 20% of the beta cells functioning, but will progress to insulindependent diabetes.…”
